For your convenience, RSVP’s Driver Activity Report is available here as a fillable field pdf form.
You can:
- Download the form and save it to your computer. Keep this original as your master form. Then each month, open the original and use “Save As” to rename the file for the reporting month. Then type your information into the fillable fields, save and email the file to the email address on the bottom of the form.
- Or, download the form, print it, fill it out by hand, and mail it to the address on the bottom of the form.
We appreciate getting reports that are filled out electronically because they are clearer and easier to read. More information on completing forms is below.
